Seeking public feedback on Healthy Lifestyles/Risk Reduction measures

MN Community Measurement (MNCM) and the measure development work group for Healthy Lifestyles/Risk Reduction is seeking feedback on a new, proposed set of measures. These measures have been developed under a contractual agreement with the Minnesota Department of Health (MDH) to support a CDC funded Community Transformation Grant. The scope of measure development exploration focused on quantifiable ways to measure and impact modifiable risk factors (blood pressure, BMI, cholesterol and tobacco use) for reducing risk of cardiovascular disease and promoting healthier lifestyles for adults.

Although measure development activities, and any future pilot testing of measures, are currently focused on the 22 counties (and one tribal community) located in Northern and Western MN that are within the CTG grant, we are seeking feedback from all clinics in MN who have the following specialties: Family Medicine, Internal Medicine, Geriatric Medicine and Obstetrics/Gynecology. Currently, there is no plan for wide implementation of any potential measures beyond the areas impacted by the grant, nor public reporting.


Primary Measure

  • Optimal Risk Reduction (BP < 140/90 and Tobacco-free and BMI < 25.0)

Secondary Measures for Subsets of the Denominator

  • Blood Pressure Elevation without Diagnosis HTN (Internal measure for QI purposes only)
  • Tobacco Cessation Counseling Intervention
