PrimeWest is a County Based Purchasing health plan serving low-income Medicaid and Medicare residents in 10 rural Minnesota counties. PrimeWest integrates publicly funded health and social services with private health care services to provide its 10,000 members with high-quality, coordinated health coverage.
South Country Health Alliance (SCHA) is a County Based Purchasing organization, operated by a group of 14 counties that purchases health care services for certain residents enrolled in Medical Assistance, General Assistance Medical Care, MinnesotaCare and Medicare products. CBP presents an enhanced alternative to the fee-for-service state-purchased and operated public programs and to the Prepaid Medical Assistance Program.
UCare is an independent, non-profit health plan, serving nearly 500,000 members. Founded in 1984, UCare provides coverage for enrollees in Medical Assistance, Medicare, and MNsure. Our members have access to a large network of health care providers.

Ortonville Area Health Services (OAHS) is a rural health care system comprised of the following facilities: Ortonville Hospital is a 25-bed critical access hospital with 4 bassinets and is designated as a Level 4 Trauma hospital and an Acute Stroke Ready hospital; Fairway View Neighborhoods is a 51-bed skilled nursing facility with 15 private resident rooms and two semi-private resident rooms within three neighborhoods; OAHS Home Health Agency is a hospital-based home health agency offering medical and non-medical patient visits; Northside Medical Clinic and Clinton Clinic are provider-based clinics staffed by six physicians and 5 mid-level providers; and Fairway View Senior Communities is a senior living facility consisting of 46 catered living apartments, 16 assisted living apartments and six memory care studio apartments. MNCM thanks our Data Contributors across Minnesota for their participation in improving public health by publicly reporting data. More than 1,450 medical groups and clinics in Minnesota and the surrounding region supply data to MNCM on quality, patient outcomes, and patient experience.
Additionally, data is supplied by our six health plan founding members (Blue Cross and Blue Shield of Minnesota, HealthPartners, Medica, Metropolitan Health Plan, PreferredOne and UCare) as well as PrimeWest Health System and South Country Health Alliance.
When you become a data contributor, you gain access to tools and resources that can benefit your patients and members when used in combination with improvement efforts. Recently added tools include race-ethnicity-language data and detailed patient experience results for participating medical groups and clinics.
