CKD Learning Collaborative: Strategies to Improve CKD Prevention & Treatment in Primary Care

Join us on Monday, May 20th from 12-1pm for the first Chronic Kidney Disease (CKD) learning collaborative! The CKD learning collaboratives are part of MNCM’s CKD Dashboard Project, which is available to all medical groups onboarded into MNCM’s PIPE system. However, this session is open to all medical groups who submit data to MNCM.

During this session, Katelyn Laue, Senior Director of Program Development from the National Kidney Foundation (NKF), will share information about NKF’s CKD Change Package Resource. Katelyn will also highlight some successful strategies in CKD prevention and treatment that NKF has learned about from primary care in states outside of Minnesota over the past few years.

Jess Donovan will also provide some brief updates for current participants regarding timelines of data refreshes and features of the dashboard.
